For your convenience, you can download and fill out the information forms (below) at home and then bring them with you on your first visit.

If you are currently under the care of a chiropractor and are traveling through Rock Hill or vacationing in the area, and need chiropractic care, we can accomodate you. Please call the office to make an appointment and give us your chiroprator's name and number so we can get up to date on your care history before the time of your appointment. We can usually see you on a same day or next day basis.
Bring the completed Forms which will save time during your first visit.
If you have spinal x-rays, please bring them with you. If they meet the following critieria, we may be able to use them instead of taking new images.
Be recent.
You have not had an accident after the films were taken.
They were taken with you standing. Please refer to Why We Use Standing X-Rays For Spinal Analysis for why this is important.
They are of good clarity and show the area of interest without obstruction. This will be determined during the office visit.
